We are thankful to Dr. Abu-Zidan for his comments [1] about our article published in the World Journal of Surgery [2]. It is clear that the exact diagnosis of epiploic appendagitis (EA) should be based on histopathologic examination. However, that requires surgical treatment which might be considered unnecessary intervention for both diagnosis and treatment of this pathology. There are some pitfalls to the design of a diagnostic algorithm for EA, including difficulties in detecting true-positive and false-negative cases. We tried to include patients from our Radiology Department and Hospital Information Systems to overcome the situation. …
