Sir: Recently I was asked to provide a pre-review opinion on a manuscript which had been submitted to the open-access journal “BioMed Central Infectious Diseases” (BMC). The editor was concerned that the study was not sufficiently different from two previous publications by some of the same authors; one in 2005 in “Clinical and Experimental Immunology” [
1] with ten authors, and one in 2006 in “Intensive Care Medicine” with nine authors, five of whose names appeared in the first publication [
