A 40-year-old female patient, with a history of chronic allergic rhinosinusitis, presented to the emergency department with a 1-day history of a painless horizontal binocular diplopia on left-sided gaze. Neurological examination revealed an isolated limited abduction of the left eye, consistent with left abducens nerve paresis.
