We read with deep interest the article by Driss et al. [
1] who emphasize the association of a cotyledonoid dissecting leiomyoma with endosalpigiosis. Cotyledonoid dissecting leiomyoma is an extremely rare variant of uterine leiomyoma with just 24 cases on record [
2]. Clinicoradiological differentiation from leiomyoma is difficult, and diagnosis is most often not suspected preoperatively. Imaging modalities offer no help, and its bizarre macroscopic shape may be confused with malignant neoplasm [
