Due to the presence of vomiting and failure to thrive, renal tubular disorders, gastrointestinal disorders, and endocrinologic abnormalities were considered in the differential diagnosis. Failure to thrive, polyuria, mild hyponatremia, and mild hyperkalemia were suggestive of renal tubular disorders, but she had normal serum phosphate and calcium levels, normal blood gases, and no glucosuria or proteinuria.
