The presacral venous plexus lies posterior to the rectal fascia and underneath the presacral fascia, presenting in a ladder-like fashion [1]. When performing total mesorectal excision (TME), iatrogenic injury may cause massive presacral hemorrhage. Although presacral bleeding is uncommon, it can be life-threatening [2]. …
