Massive localized lymphedema (MLL) was first reported in 1998 and has been described after surgery and blunt trauma and in morbidly obese patients [
1‐
4]. The etiology of MLL remains unclear but it is almost solely described in obese patients. Local obstruction of lymph vessels in the skin due to excessive fat tissue may cause progressive growth of the cutaneous lesion. Traction together with lymphoedema can induce local skin ischemia which might be the cause of the frequently seen ulcers [
5]. Differential diagnosis includes giant inguinal-, umbilical- or scrotal herniation, massive lipoma, excessive adipose tissue and (malignant) neoplasm such as a well-differentiated liposarcoma (WDL). …
