We investigated a 55-year-old Chinese female for persistent mild neutropenia. Apart from weight loss of 3 kg over the past year, she was otherwise well with no abnormal physical findings. Full blood count showed white blood cell (WBC) count 5.44 × 109/L, absolute neutrophils 1.8 × 109/L, absolute lymphocyte 3.1 × 109/L, hemoglobin concentration 11.9 g/dL, and platelet count 306 × 109/L. Blood film showed large granular lymphocytes (LGL) (4 %) (Fig. 1a, b). Screening tests for autoimmune disorders, Epstein–Barr virus infection, and HIV infection were negative. Chest X-ray and ultrasound scan of the abdomen were normal, except for stones in the gallbladder.
