Perihilar cholangiocarcinoma (PHC) represents a major challenge for surgeons because of several perioperative conditions: preoperative jaundice management and risk of infections, evaluation of tumor extension and assessment of the future liver remnant (FLR), and risk of post-hepatectomy liver failure (PHLF) due to the cholestatic liver. All these conditions increase the risk of perioperative mortality, which exceeds 15% in some series—posing the urgency for novel standards of care.1 In addition, the complexity of biliary resection and reconstruction limited the diffusion of minimally invasive strategies so far. …
