Changing the Rules of Resectability of T4b Esophageal Cancer Invading Aorta

We read with great interest the recent publication of Matsuda et al. [1] demonstrating a large retrospective study of Siewert type II esophagogastric junction (EGJ) carcinomas. The striking finding of this study was that para-aortic and mediastinal lymph node metastasis (above 16%) and recurrence rates (37.8%) were relatively high. Of interest, also, was the finding that around 30% of the cases were staged as T4 without further subclassification or any subgroup analysis about the adjacent tissues that were invaded. Moreover, it is not so clear, how many patients with T4 disease were finally offered a R1 resection. …
