A 52-year-old epileptic woman—under oxcarbazepine and lamotrigine—was referred to the emergency room because of the sudden onset of nystagmus, ataxia, dizziness, acute renal failure and tachycardia. Given the pathological feature of EKG, an intravenous bolus of 50 mL of sodium bicarbonate 8.4 % was administered and immediately resulted in restoration of normal cardiac rhythm (Fig. 1).
