Bimaxillary surgical treatment in a young adult withsevere protrusion and compromised periodontium

This article describes the treatment of a periodontally compromised adult male with a convex profile and proclined upper and lower incisors with severe lip incompetency. The treatment protocol included the following stages: (1) periodontal flap surgery and root planning with regular follow up, (2) presurgical orthodontics leveling and aligning of the maxillary and mandibular arches with closure of all spaces, (3) surgical phase with anterior maxillary osteotomy and mandibular subapical osteotomy and (4) post-surgical orthodontics for finishing and detailing. The treatment lasted 16 months and resulted in a drastic facial improvement.
