In mechanical thrombectomy (MT), balloon guide catheters (BGC) are often used in the anterior circulation to effectively achieve flow arrest and reversal during stent-retriever (SR) retraction in order to avoid distal clot embolization [1, 2]. The complexity of standard MT procedures may be increased due to upstream high-grade proximal internal carotid artery (ICA) stenosis requiring stenting and/or extremely tortuous vessel anatomy. Under these circumstances, stable access and passage of distal access catheters (DAC) and microcatheters seem to be crucial determinants of procedural success. …
