The treatment strategy for pancreatic ductal adenocarcinoma (PDAC) has been decided according to resectability status.1,2 Tumor size (TS) is a simple but definite prognostic factor and has been employed to determine the T factor of the TNM staging system.3 Resectability status, however, defined on the basis of only anatomic tumor extension, especially in relation with the surrounding major vessels and TS, is not accounted for. There is a concern regarding whether all patients with the same resectability status should be treated uniformly irrespective of TS. …
