ASO Author Reflections: Not All Low-Grade Appendiceal Mucinous Neoplasm Mucin is Created Equal

Taken as a whole, patients with peritoneal dissemination of low-grade appendiceal mucinous neoplasms (LAMNs) treated with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) have a favorable prognosis.1,2 However, some of these patients recur even after complete cytoreduction, and mucin composition and distribution may explain the difference in prognosis. Patients with peritoneal carcinomatosis of LAMN can have two types of peritoneal deposits containing acellular mucin (AM) or cellular mucin (CM) when epithelial cells are present.2 Although the American Joint Committee on Cancer 8th edition classifies both types of LAMN peritoneal spread as one prognostic group (stage IVa), there is some data that peritoneal lesions with AM and CM might affect survival differently.2,3 Due to the rarity of the disease, this data is based on studies in mixed populations, including recurrent patients.3,4 The impact of mucin cellularity on progression-free survival (PFS) and overall survival (OS) in newly diagnosed LAMN patients has not been reported. …
