The renal biopsy findings were segmental fibrinoid necrosis, nuclear debris, and neutrophil leukocytic infiltration in 18 of 24 glomeruli (Fig. 1). There were epithelial crescents in 3 glomeruli. Tubular atrophy and microcalcification in tubular lumens were observed. Immunofluorescence showed faint granular IgA and fibrinogen staining along the glomerular basement membrane. The renal biopsy was diagnosed as a necrotizing glomerulonephritis. Histological evaluation of the intestinal resection material showed transmural inflammation of the small and medium-sized vessels with a heavy infiltrate of neutrophils and mononuclear cells accompanied by nuclear debris and fibrinoid necrosis of the vessel wall. An inflammatory reaction was also observed around the small and medium-sized vessels, and in the omentum.×
