A 20-year-old woman with a long history of chronic kidney disease resulting from a myeloperoxidase anti-neutrophil cytoplasmic autoantibody (ANCA)-positive crescentic glomerular nephritis developed increasing serum calcium levels in the summer of 2012, 10 years after her original diagnosis. She had initially been treated with cyclophosphamide and glucocorticoids and after some years was maintained on mycophenolate mofetil alone. Her ANCA serologies had been negative for more than 6 years after a single disease recurrence. Her serum creatinine had been stable at over 4 mg/dL (estimated glomerular filtration rate < 20 mL/min/1.73 m2) for several years. She was on the waiting list for a kidney transplant. …
