The treatment of ARDS has been and still is proving challenging [1]. The ARDSnet study showed mortality benefits in ventilating patients using low tidal volumes 6 ml/kg (of predicted weight) compared to a “traditional” approach using 12 ml/kg [2]. While the message that low tidal volume ventilation is advantageous has filtered down into clinical practice, the importance of basing ventilation volumes on a predicted weight seems to have been filtered out. …
