We thank Drs. Hussein and El-Hasani for their interest in our article and their insightful comments regarding their approach to mesh insertion during laparoscopic ventral hernia repair. Any incision made for mesh insertion raises concern about potential mesh contamination. The authors’ suggested placement of a trocar through the hernia defect for mesh insertion is a valid approach that certainly prevents contact of the mesh with the skin flora. This technique, however, requires placement of an additional trocar and mesh insertion through this port, subsequently increasing the procedure time and cost. Furthermore, they suggest that this trocar may be used for omental extraction, a maneuver likely to prove technically difficult. …
