The result was an artifact. The falsely elevated creatinine level was due to the presence of nitromethane, a component of model racing fuel. The active methylene group of nitromethane reacts with the alkaline picrate of the Jaffe reaction in a similar manner to creatinine producing a similar end product and falsely elevating the measured creatinine level.

The patient’s other laboratory studies as well as his clinical condition were not consistent with significant methanol toxicity. In addition, the patient did not demonstrate any additional physical examination findings, such as edema or hypertension, or any laboratory findings, such as metabolic acidosis, hyperkalemia, azotemia, and anemia that would be expected in the setting of acute kidney injury with a serum creatinine of 8.6 mg/dl.
