Abstract.
After positive mesenteric angiography for massive lower gastrointestinal hemorrhage, one is faced with a localized bleeding site and the therapeutic options of embolization, vasopressin infusion, observation, or operation. This review was designed to determine long term outcomes of angiographically controlled bleeding. All cases of mesenteric angiography for hemorrhage performed over a twelve year period were reviewed, with focus on those treated non-operatively. A total of 37 patients had angiographically localized bleeding distal to the ligament of Treitz. Twenty-one patients were controlled with vasopressin, embolization, or spontaneous cessation. Only three patients had recurrent bleeding, at one month, one year, and eight years. No patients died from recurrent bleeding. Five patients died without any further bleeding; mean time to death was 2 years. Twelve patients had no further bleeding at a mean follow-up of 2.6 years. Bleeding controlled by any angiographic measure, was followed by recurrent bleeding in 14% without the need for operative intervention.

Si l'angiographie est positive, en cas d'hémorragie gastro-intestinale basse et massive, l'on est confrontéà un foyer hémorragique localisé pouvant être traité par des options différentes: embolisation, infusion de vasopressine, surveillance ou opération. Le but de cette étude est de déterminer le devenir à long terme de foyers hémorragiques contrôlés par l'angiographie. Toutes les angiographies mésentériques réalisées pour hémorragie au cours d'une période de 12 ans ont été revues, en particulier celles faites en cas d'hémorragie traitée non chirurgicalement. Chez 37 patients, le foyer hémorragique a été localiséà l'angiograhie en aval du ligament de Treitz. Vingt-et-une fois le saignement a été contrôlé par vasopressine, embolisation ou a cessé spontanément. Seuls trois patients ont développé des saignements itératifs à, respectivement, un mois, une année et 8 ans. Aucun malade n'est décédé de saignements itératifs. Cinq patients sont décédés sans reprise de l'hémorragie, en moyenne après deux ans. Douze patients n'ont présenté aucun saignement secondaire au cours d'un suivi moyen de 2,6 ans. Le saignement contrôlé par angiographie n'a entraîné un saignement secondaire que dans 14% des cas sans qu'un geste chirurgical n'ait été nécessaire.
