Zusammenfassung
Hintergrund
Stress gilt bekanntlich als Risikofaktor für einen akuten Myokardinfarkt und den plötzlichen Herztod. Eine spezielle Form der durch akute starke psychische und/oder physische Belastung ausgelösten Kardiomyopathien stellt die Tako-Tsubo-Kardiomyopathie (TTK) dar.
Ziel der Arbeit
Die pathomorphologischen und rechtsmedizinischen Aspekte dieses seltenen Krankheitsbilds werden beschrieben. Besonderes Augenmerk liegt darauf, den Leser für bestimmte Befundkonstellationen, die für die korrekte Diagnosestellung bedeutsam sind, zu sensibilisieren.
Material und Methode
Der vorliegende kasuistische Beitrag stellt 5 verschiedene und nicht unbedingt typische Fälle von überlebten sowie nichtüberlebten Komplikationen der TTK vor.
Ergebnisse
Todesursächlich bei dieser zunehmend häufiger diagnostizierten, jedoch immer noch seltenen Erkrankung sind die in bis zu 3 % der Fälle auftretenden Komplikationen. Ohne die kardiologische Vorgeschichte der Patienten zu kennen, sind die Patienten schwer der korrekten Diagnose zuzuordnen, da die Komplikationen auch im Rahmen von anderen häufigeren kardiologischen Erkrankungen auftreten.
Schlussfolgerung
Autoptisch ist es von großer Bedeutung, bestimmte Befundkonstellationen zu erkennen und richtig zu deuten, um in der Zusammenschau mit der Vorgeschichte bzw. Anamnese des Betroffenen die korrekte Todesursache zu verifizieren. Bei Vorliegen einer TTK sind ggf. die Angehörigen aufzuklären und bei einer potenziellen genetischen Disposition der Erkrankung einer kardiologischen Vorsorge zuzuführen. Forensische Aspekte dieser Erkrankung sind insbesondere bei plötzlichen Todesfällen relevant, da auslösende exogene Triggerfaktoren die Pathogenese beeinflussen.
Abstract
Background
Stress is a well-known risk factor for acute myocardial infarction and sudden cardiac death. Takotsubo cardiomyopathy represents a special form of cardiomyopathy triggered by acute severe emotional and/or physical stress.
Aim
The pathomorphological and medicolegal aspects of this rare disorder are described. Particular attention is paid to make the reader aware of certain constellations of findings which are important for a correct diagnosis.
Material and methods
This case study article presents five different and not necessarily typical cases of survived and non-survived complications of takotsubo cardiomyopathy.
Results
The cause of death in this increasingly more frequently diagnosed but still rare disease is due to the occurrence of complications in up to 3 % of cases. Without knowing the cardiological medical history of the patients it is difficult to assign the correct diagnosis to the patients because the complications can also occur in other more frequent cardiological diseases.
Conclusion
At autopsy it is of great imortance to recognize and correctly interpret certain constellations of findings and to include these in the summary together with the background and medical history in order to verify the correct cause of death. In the case of takotsubo cardiomyopathy relatives must be adequately informed if necessary and prophylactic cardiological measures must be undertaken if there is a potential genetic predisposition for the disease. Forensic aspects of this disease are particularly relevant in cases of sudden death because the triggering exogenic factors influence the pathogenesis.
