Zusammenfassung

Der primäre Hyperaldosteronismus (PHA) ist die am häufigsten vorkommende heilbare Ursache von Bluthochdruck mit einer Prävalenz bei Hypertonikern bis zu 12%. Die selektive Blutentnahme aus der Nebennierenvene (NNV) gilt als diagnostischer Goldstandard. Sie wird jedoch aufgrund einer hohen Fehlerquote oftmals nicht genutzt. Die Anwendung der C-Arm-CT während der Blutentnahme kann diese Fehlerquote reduzieren bzw. eliminieren. Wird während der Intervention unter Durchleuchtung die C-Arm-CT eingesetzt, um die Katheterlage vor der Blutentnahme zu prüfen, verbessert dies den technischen Erfolg des Verfahrens erheblich. Etabliert sich dieses Verfahren, bedeutet dies eine Verbesserung von Diagnose und Behandlung der Patienten mit primärem Hyperaldosteronismus.

Abstract

Primary hyperaldosteronism is the most common curable cause of hypertension with a prevalence of up to 12% among patients with hypertension. Selective blood sampling from adrenal veins is considered the diagnostic gold standard. However, it is underutilized due to the high technical failure rate. The use of C-arm CT during the sampling procedure can reduce or even eliminate this failure rate. If adrenal vein sampling is augmented by native C-arm CT to check for the correct catheter position, the technical success rate increases substantially. General use of this technique will result in correct diagnosis and treatment for patients with primary hyperaldosteronism.
