Zusammenfassung
Studienziel
Ziel der prospektiven Studie war es zu untersuchen, ob SaluCartilage-Hydrogelzylinder eine mögliche Alternative bei der Behandlung von schweren Knorpelschäden sind.
Material und Methode
Die prospektive Studie wurde klinisch, röntgenologisch sowie magnetresonanztomographisch bei 18 Patienten mit Grad-IV-Knorpelschäden, bei denen im Zeitraum von Oktober 2002 bis Juli 2003 49 Hydrogelzylinder implantiert wurden, durchgeführt. Die klinische Beurteilung erfolgte mit dem Score nach McDermott.
Ergebnisse
Nach 3 Monaten postoperativ wurde eine signifikante Verbesserung des McDermott-Scores auf 75 Punkte (p<0,05), nach 6 Monaten postoperativ auf 80 Punkte (p<0,05) beobachtet, der aber nach 12 Monaten auf 69 Punkte gefallen war. Bei den MRT-Nachuntersuchungen nach 3 und 6 Monaten zeigt sich bei allen Patienten um die Hydrogelzylinder ein Flüssigkeitssaum mit fehlendem Einwachsen ohne Dislokation. 12 Monate postoperativ waren bei 2 Patienten die Hydrogelzylinder disloziert und zerbrochen.
Schlussfolgerung
Im Rahmen dieser frühen Nachuntersuchung ist eine subjektive und objektive Verbesserung des Beschwerdebildes, wahrscheinlich aufgrund der Platzhalterfunktion nachweisbar. Aufgrund der Ergebnisse unserer Studie ist diese Operationsmethode insbesondere aufgrund des ungenügenden Einwachsverhaltens mit Rotationsmöglichkeit und Dislokationsgefahr sehr kritisch zu beurteilen.
Abstract
Aim of study
This prospective report presents SaluCartilage hydrogel implants, an alternative therapy option in the treatment of chondral defects.
Methods
For the prospective study between October 2002 and July 2003, 18 patients suffering from stage IV chondral defects received 49 hydrogel implants and were subsequently examined by means of clinical, radiological, and magnetic resonance imaging assessment. The knee function was classified using the McDermott score.
Results
The average McDermott score was improved to 75 points (p<0.05) after 3 months postoperatively and to 80 points (p<0.05) 6 months postoperatively, but after 12 months it decreased to 69 points. After 3 and 6 months, the MRI showed in all patients that there was fluid around the hydrogel implants without dislocation. After 12 months postoperatively the hydrogel implants had been destroyed in two patients.
Conclusion
The short-term results showed subjective and objective improvement of the medical conditions probably because of the placeholder function. This alternative should be critically discussed because of the inadequate connection to the bone with risk of dislocation.
