Zusammenfassung
Anamnese
Wir berichten über eine 49-jährige Patientin die sich mit einer seit circa 1,5 Jahren bestehenden Dysphagie für flüssige und feste Kost vorstellte. Zusätzlich traten seit einem Jahr beim Verzehr von fester Nahrung Ohnmachtsanfälle auf. Die Beschwerden waren begleitet von einem Globusgefühl und Regurgitationen.
Klinischer Befund
Die HNO-ärztliche Spiegeluntersuchung zeige eine Zungengrundhyperplasie und Rötung der Aryregion. In der Ösophagogastroduodenoskopie (ÖGD) konnte der Befund einer großen (> 3 cm) axialen Hiatushernie gesichert werden. Eine Ösophagusmanometrie zeigte einen diffusen Ösophagusspasmus. Im Langzeit-EKG ergab sich ein intermittierender AV-Block 3. Grades.
Therapie und Verlauf
Nach Impantation eines Herzschrittmachers traten keine erneuten Ohnmachtsanfälle auf. Ein Globusgefühl und Regurgitationen bestanden weiterhin. Eine Besserung erbrachten Protonenpumpeninhibitoren. Postinterventionell fand sich in der funktionellen endoskopischen Untersuchung des Schluckvorgangs ein regelrechter Befund.
Schlussfolgerung
Synkopen infolge der regulären Nahrungsaufnahme sind eine extreme und seltene Form der vasovagalen Reflexanwort. Eine interdisziplinäre Abklärung durch HNO-Arzt, Neurologen und Internisten ist erforderlich.
Abstract
Medical history
A 49-year-old woman was referred to the ENT department with a 1.5-year history of dysphagia accompanied by globus sensation and regurgitation as well as recurrent syncopal attacks associated with swallowing solid foods.
Clinical findings
Endoscopic examination revealed hyperplasia of the tongue base and redness of the aryepiglottic region. An axial hiatal hernia (> 3 cm) was seen on esophago-gastro-duodenoscopy. Esophageal manometry showed a diffuse esophageal spasm. Holter monitoring (24 h) revealed a third-degree atrioventricular block.
Therapy
The patient’s symptoms resolved following placement of a permanent cardiac pacemaker. Globus sensation and regurgitation persisted. Treatment with proton pump inhibitors was successful. The final functional endoscopic evaluation of swallowing revealed no relevant findings.
Conclusion
We describe a case of a rare and extreme form of a vagal reflex. Deglutition syncope can be successfully treated by a multidisciplinary team consisting of otorhinolaryngologists, neurologists and cardiologists.
