Zusammenfassung
Hintergrund und Ziel:
Im Frühstadium der akuten Pankreatitis werden neben Faktoren des Entzündungs- und Gerinnungssystems Sauerstoffradikale freigesetzt, die zu einer Imbalance zwischen Oxidanzien und Antioxidanzien führen können. Tierexperimentell konnte durch Selen als Radikalfänger eine Verringerung der Radikalwirkung erreicht werden. Mitteilungen klinischer Ergebnisse sind widersprüchlich. Im Rahmen einer prospektiven, randomisierten Blindstudie sollte der Effekt einer standardisierten Hochdosis-Natriumselenit-Therapie an Patienten mit akuter Pankreatitis untersucht werden.
Patienten und Methodik:
Prospektive Randomisierung von 70 Patienten mit akuter Pankreatitis in Selengruppe (standardisiertes Selenschema) und Plazebogruppe (0,9% Natriumchlorid). Untersuchung klinischer und paraklinischer Parameter zum Verlauf der Erkrankung bei Aufnahme und in den Tagen danach.
Ergebnisse:
Der klinische Verlauf unterschied sich in beiden Behandlungsgruppen nicht signifikant. Fünf Patienten der Selengruppe und drei Patienten der Plazebogruppe verstarben. Laborchemisch stieg unter Substitution die Selenkonzentration signifikant an. Die Serumspiegel der Glutathionperoxidase und des Malondialdehyds wiesen jedoch für die Behandlungs- und Plazebogruppe keine signifikanten Unterschiede auf.
Schlussfolgerung:
Eine Empfehlung zur adjuvanten Substitution mit Natriumselenit bei akuter Pankreatitis lässt sich aus den Ergebnissen nicht ableiten.
Abstract
Background and Purpose:
Oxygen free radicals appear in the early phase of acute pancreatitis followed by an imbalance between the oxygen and anti-oxygen system. Sodium selenite may play an important role in the reduction of radicals in experimental pancreatitis. The aim of this study was to evaluate the effects of selenium in a clinical prospective trial.
Patients and Methods:
70 patients with acute pancreatitis were prospectively allocated to a selenium and a placebo group. Clinical and paraclinical parameters were investigated with a follow-up of 90 days after hospital stay.
Results:
The clinical course of the two treatment groups did not show statistically significant differences. Five patients in the selenium and three patients in the placebo group died. Depending on selenium therapy, antioxidant substances were presented in higher levels. All 17 clinical parameters showed no statistical significance.
Conclusion:
Substitution of sodium selenite has no beneficial effect on the clinical outcome of patients with acute pancreatitis.
