Zusammenfassung
Hintergrund
Bei komplizierten inferioren Netzhautablösungen haben wir Fluoron als mittelfristige Tamponade verwendet. Diese Arbeit untersucht die Vorteile und Nachteile dieses neuen Instrumentariums in der Chirurgie des hinteren Abschnittes.
Material und Methode
In einem Zeitraum von 11 Monaten wurde Fluoron, ein flüssiges teilfluoriertes Alkan [5, 6] bei 14 Patienten mit komplizierter Pathologie der inferioren Netzhaut verwendet. Intraoperativ verliefen die Operationen komplikationslos. F6H8 wurde bis zu 3 Monaten im Auge belassen.
Ergebnisse
Bei 8 Augen konnte eine dauerhafte Anlage der Netzhaut nach Entfernung des Fluorons erreicht werden. Bei 1 Patienten liegt die Netzhaut derzeit an, das Fluoron befindet sich jedoch noch im Auge. Bei 6 Augen trat eine Reamotio auf. Die beobachteten mögliche Nebenwirkungen bzw. Komplikationen umfassten starke Photophobie, Schmerzen, bleibende Hypotonie, vorzeitige Blasenbildung.
Schlussfolgerung
Fluoron stellt von Seiten der Tamponadewirkung und der intraoperativen Anwendung eine Ergänzung im bisherigen Instrumentarium der komplizierten Netzhautchirurgie dar, auf Grund unerwünschter Wirkungen sollte die Indikation streng sein (als ultimo ratio anzuwenden), die Entfernung möglichst früh erfolgen (nach 1–2 Monaten).
Summary
Complicated detachments of the inferior retina are still a challenge in posterior segment surgery. Therefore Fluoron could be an approbiate medium-termed tamponade agent. This study examines the advantages and disadvantages of this new tool in vitreoretinal surgery.
Materials and methods
During 11 months 14 patients with complicated pathology of the inferior retina were treated with Fluoron. This agent belongs to the semifluorinated alkanes [5, 6]. During the Operations no complications occurred. Fluoron remained within the eye up to 3 months.
Results
In 8 eyes the retina remained reattached, even after removal of fluoron, in six eyes redetachment occurred. In one patient the retina is still reattached, but the fluoron is not removed yet. We found side effects (complications) possibly due to the fluoron like severe photophobia, pain, hypotonia or early emulsification.
Conclusions
Fluoron is a new medium term tamponade in complicated vitreoretinal surgery. Due to side effects like pain, hypotonia, corneal opacifications that may be induced by this new agent, fluoron should be restricted to complicated reattachment with pathological changes in the inferior retina, removal should be as early as possible.
