Inleiding Als reactie op de door huisartsen in een drietal gezondheidscentra in Maastricht ervaren noodzaak het zeer diverse en groeiende probleemaanbod beter te structureren, hebben we een literatuuronderzoek gedaan naar het bestaan van gevalideerde triagesystemen en de effecten hiervan.
Methoden Er is een systematisch literatuuronderzoek uitgevoerd door raadpleging van MEDLINE, EMBASE en Cochrane Library.
Resultaten Er zijn negen onderzoeken geïncludeerd, waarvan één review en acht trials.
De drie beschreven triagesystemen betreffen respectievelijk triage door de huisarts, door de praktijkverpleegkundige en door de praktijkassistente. De huisarts kan door zelf telefonisch te triageren 50 tot 72% van de consultverzoeken exclusief telefonisch afhandelen. Op langere termijn resulteert dit in 39% minder praktijkconsulten. Triage door geschoolde verpleegkundigen (in combinatie met een kleinekwalenspreekuur) levert een reductie van 29 tot 40% van de praktijkconsulten door de huisarts op. Het beperkte Nederlandse onderzoek toonde bij triage door praktijkassistenten (in combinatie met een assistentenspreekuur) een reductie van 10% van de praktijkconsulten.
Beschouwing Triage door huisartsen of verpleegkundigen is een effectieve en waarschijnlijk veilige methode om het aantal praktijkconsulten door de huisarts te verminderen. Hoewel er al triagesystemen met assistenten in gebruik zijn, is er in de literatuur nog onvoldoende bewijs voor de effectiviteit van triage door assistenten.

Background In response to the increased demand for GP time in a group of primary care surgery sites in Maastricht (the Netherlands), new methods of practice management and organisation are needed to keep up with demand. The objective of this review is to compare and investigate the different day-time triage systems found in literature.
Method We systematically searched MEDLINE, Cochrane and EMBASE.
Results Nine studies met our criteria; one review and eight trials. GPs can handle 50-72% of the requests for same-day appointments exclusively by telephone. Ultimately demand for face to face appointments was reduced by 39%. Nurse telephone triage combined with a minor illness nurse clinic reduced the face to face GP appointments by 29-40%. Scarce Dutch research showed a 10% reduction in the number of GP appointments in a doctors-assistant led triage system (combined with minor illness clinic).
Conclusion Both triage by GPs and nurses are effective and probably safe systems to reduce doctor’s workload. Although triage by practice assistants already is in use in several places, literature shows insufficient proof of effectiveness of such systems.
