Zusammenfassung
Von den zahlreichen in der Literatur angegebenen Methoden zur Behandlung eines chronischen Subduralhämatoms wurde bei 51 Patienten die der externen geschlossenen Drainagebehandlung nach Bohrlochtrepanation gewählt. Die Technik ist einfach, schnell, in Lokalanästhesie durchführbar und beschränkt sich auf das Ausspülen des Hämatoms und die Einlage von 1 bis 2 Drainagen ohne Kapselresektion. Bei der Entlassung nach durchschnittlich 13 Tagen war die Hälfte der Patienten symptom- und beschwerdefrei, 22 von 51 noch mit geringen Ausfällen und lediglich zwei mit deutlicheren Ausfällen behaftet. Ein Patient verstarb an bereits präoperativ manifesten Sekundärkomplikationen. Nachteile, die sich letztlich nicht im Ergebnis niederschlagen, sind eine relativ lange stationäre Behandlungsdauer und Reoperationen wegen gekammerter Hämatome oder verstopfter Schläuche.
Abstract
For the treatment of chronic subdural hematoma numerous methods are known. 51 patients were treated with external closed drainage after bore-hole trepanation. The technic is simple and easy; it can be done under local anesthesia. The hematoma is washed out and one or two drainage-tubes are inserted without resecting the capsule. The patients could be discharged in average after 13 days; at this time half of the patients had no symptoms or any complaints. 22 of 51 patients showed only slight neurological defects, two had severe defects. One patient died due to seondary complications, which had been evident already before surgery. Disadvantages of the method are a relatively long stay in hospital; sometimes reoperations are needed because of localized hematomas or blocked drainage-tubes.
