Summary
Twenty-four hours after oral administration of metformin-14C to mice (100 mg/kg), an average of 77.2% of the administered activity was recovered in the urine and 8.4% in the faeces. Under the same test conditions the rat eliminated 66.8% of the administered dose in the urine and 18.4% with the faeces. Less than 1% of the radioactivity was found in the bile. Unchanged metformin was the only radioactive substance which appeared in the urine of rats and mice. The investigation comparing the distributions of buformin-14C and of metformin-14C in the body organs gave a similar picture for both biguanides. The highest biguanide concentrations were found in the kidneys, adrenals, pancreas and liver. Serum concentrations amounted to an average of 1.2 μg/ml after oral administration of 50 mg buformin-14C/kg and 6.2 μg/ml after 150 mg metformin-14C/kg. Healthy human volunteers eliminated an average of 63% of the administered dose in the urine within 36 h. Metformin was not metabolized by the human being. From the rate of elimination in the urine, the biological half-life of the biguanide was found to be 2.8 h. Compared with other biguanides the weak hypoglycaemic activity of metformin in both animals and diabetics cannot be explained by low absorption. It is due to the weak intrinsic activity of this biguanide as such.
Résumé
On a administré à des souris par voie orale, 100 mg/kg de metformine-14C. Après 24 h on a pu retrouver on moyenne 77.2% de la dose administrée dans l'urine et 8.4% dans les fèces. Dans les mêmes conditions d'expérimentation le rat élimine 66.8% de la dose par les urines et 18.4% par les fèces. On a décelé moins de 1% de la dose radioactive dans la bile. Dans l'urine du rat et de la souris, on a trouvé uniquement la substance radioactive inchangée. Une étude comparative de la distribution dans les organes n'a pas révélé de différence entre les deux biguanides, la buformine-14C et la metformine-14C. On a trouvé les concentrations les plus élevées dans les reins, les capsules surrénales, le pancréas et le foie. Après administration d'une dose orale de 50 mg/kg de buformine-14C la concentration sérique était de l'ordre de 1.2 μg/ml en moyenne, et après administration d'une dose de 150 mg/kg de metformine, de l'ordre de 6.2 μg/ml. Des volontaires sains éliminent pendant 36 h en moyenne 63% de la dose administrée par l'urine. Également chez l'homme, la metformine n'est pas métabolisée. D'après la vitesse d'excrétion urinaire le temps de demi-vie biologique du biguanide est de 2.8 h. Comparé à celui d'autres biguanides, l'effet hypoglycémiant plus faible de la metformine aussi bien chez le diabétique que chez l'animal, ne s'explique pas par une résorption moindre. Il doit être ramené à une activité intrinsèque moins importante.

24 Std nach oraler Gabe von 100 mg/kg Metformin-14C an Mäuse lassen sich durchschnittlich 77.2% der applizierten Aktivität im Urin und 8.4% im Kot nachweisen. Die Ratte eliminiert unter denselben Versuchsbedingungen 66.8% der Dosis mit dem Harn und 18.4% mit dem Kot. In der Galle findet sich weniger als 1% der applizierten Radioaktivität. Im Urin von Ratten und Mäusen tritt als einzige radioaktive Substanz unverändertes Metformin auf. Die vergleichende Untersuchung der Organverteilung von Buformin-14C und Metformin-14C ergibt für beide Biguanide ein ähnliches Bild. Die höchsten Biguanidkonzentrationen finden sich in Niere, Nebenniere, Pankreas und Leber. Die Serumkonzentrationen betragen nach oraler Gabe von 50 mg/kg Buformin-14C durchschnittlich 1.2 μg/ml und nach 150 mg/kg Metformin-14C 6.2 μg/ml. Gesunde Probanden eliminieren innerhalb von 36 Std durchschnittlich 63% der applizierten Dosis mit dem Harn. Metformin wird auch beim Menschen nicht metabolisiert. Aus der Ausschei-dungsgeschwindigkeit mit dem Urin ergibt sich eine biologische Halbwertzeit des Biguanids von 2.8 Std. Die beim Tier und beim Diabetiker im Vergleich zu anderen Biguaniden schwächere blutzuckersenkende Wirkung von Metformin kann nicht mit einer geringeren Resorption erklärt werden. Sie ist auf eine geringere intrinsische Aktivität dieses Biguanids zurückzuführen.
