Summary
The author investigated the case histories of 18 pairs of twins (seven cases selected from own patients, 11 cases taken from the literature).
In three discordant pairs of twins anal atresia formed a part of complex malformations (acardius amorphus, sireniform monopodia, thalidomide-induced anomalies of the upper extremities). Out of the 15 remaining pairs of twins ten pairs were of the same and five pairs of different sexes. Two out of three monocygous pairs were concordant, the others were discordant. The case histories of the concordant pairs of twins, both taken from single observations, are most likely to have been published in detail since they are rare and unusual cases. Out of ten pairs of twins, taken from serial investigations on congenital abnormalities, four can be regarded as monocygous according to the rule of Weinberg. All ten pairs oft wins were discordant. Discordance appears to be the rule in twins with anal atresia.
When comparing the findings in twins with observations in families it becomes obvious that genetic components play only a minor part in the etiology of anal atresia.
Amongst patients with anal atresia, twins have been found somewhat more frequently than among total births. It seems noteworthy to point towards the remarkable combination of anal atresia with hypo- or aplasia of the thumb and anomalies of the vertebrae and ribs. These facts are discussed and compared to a similar pattern of malformations which only differs from the above mentioned pattern insofar as the aplasia of the thumb is replaced by a duplication of it.

Wir haben 18 Zwillingspaare mit Analatresie (7 eigene, 11 Literaturfälle) zusammengestellt.
Bei 3 diskordanten Paaren war die Analatresie Teil komplexer Mißbildungen (Acardius amorphus, sirenoide Monopodie, conterganbedingte Fehlbildungen der oberen Extremitäten).
Von den verbleibenden 15 Paaren waren 5 verschieden-, die restlichen gleichgeschlechtig. Von 3 eineiigen Paaren waren 2 konkordant. Alle übrigen Paare waren diskordant.
Die beiden aus Einzelmitteilungen stammenden konkordanten Paare sind wahrscheinlich als seltene und auffällige Beobachtungen bevorzugt publiziert worden. Von 10 aus Mißbildungserhebungen stammenden Paaren können nach der Weinbergschen Regel 4 als eineiig angesehen werden. Alle 10 Paare sind diskordant. Diskordanz scheint demnach bei Zwillingen mit Analatresie die Regel zu sein.
Die Zwillingsbefunde stimmen mit Familienuntersuchungen überein, wonach die erbliche Komponente bei der Entstehung der Analatresie von untergeordneter Bedeutung ist.
Zwillingskinder wurden unter Patienten mit Analatresie etwas häufiger als unter Neugeborenen gefunden. Die bemerkenswerte Kombination von Analatresie, Hypo- oder Aplasie des Daumens, Wirbelkörper- und Rippenanomalien wird im Zusammenhang mit einem ähnlichen Mißbildungsmuster, bei dem lediglich das Fehlen des Daumens durch eine Verdoppelung ersetzt ist, diskutiert.
