Alteration of maximum anal resting pressure by digital rectal examination prior to manometry: analysis of agreement between repeat measurements

Abstract

To study whether digital examination preceding anal manometry causes significant alteration of maximum resting pressure reading and to quantify the discrepancies, 78 individuals (64 incontinent, 14 controls) were investigated. Recordings of maximum resting pressure were taken before and after digital rectal examination. There was a mean discrepancy of only-1.8 cms H2O between the readings and excellent correlation, but analysis of agreement revealed a bias that tended to be greater with smaller measurements and unacceptable variability between test results. Furthermore, the bias was not related to age, gender, the grade of incontinence, maximum voluntary contraction, functional anal canal length and threshold volume. Digital rectal examination prior to manometry causes unpredictable results especially in patients with lower maximum resting pressures and should strictly be avoided.

Résumé

Afin de déterminer si le toucher rectal précédant un examen manométrique modifie de manière significative la pression maximale de repos, 78 sujets (64 incontinents et 14 contrôles) ont fait l'objet d'investigations pour quantifier les écarts mesurés. Des enregistrements de la pression de repos maximale ont été entrepris avant et après le toucher rectal. L'écart moyen entre les mesures n'est que de-1,8 cm d'H2O et la corrélation entre les différentes mesures est excellente: une analyse statistique a toutefois mis en évidence un biais qui semble plus important avec des petites mesures et une variabilité inacceptable entre ces différentes mesures. Par ailleurs, ce biais n'est pas en relation avec l'âge, le sexe, le degré d'incontinence, la contraction maximale volontaire, la longueur fonctionnelle du canal anal et la mesure des volumes seuils. Un toucher rectal avant une manométrie peut entraîner des modifications imprédictibles des résultats, en particulier chez des patients porteurs d'une pression maximale de repos réduite, aussi l'investigation digitale devrait-elle être réduite au minimum.
