Zusammenfassung
Im Curriculum der chirurgischen Facharztausbildung hat die minimalinvasive Chirurgie (MIC) einen zunehmend bedeutenden Stellenwert. Die besonderen Schwierigkeiten zu Beginn der Lernkurve der MIC können und sollten unter kontrollierten Bedingungen außerhalb des OP erlernt werden. Dies schließt auch das Erlernen der Kameraführung mit ein. Ein wachsendes Angebot an verschiedenartigen Simulatoren und Skills Trainern bietet hierfür gute Voraussetzungen. Für einen größtmöglichen und nachhaltigen Effekt ist es sinnvoll, innerhalb der Klinik ständig die Möglichkeit für ein kontinuierliches bzw. wiederholt durchführbares Training bereit zu stellen, das idealerweise strukturiert und standardisiert ablaufen und dessen Inhalt und Ablauf validiert sein sollten. Mit wachsender Erfahrung und Fertigkeit sollten im Weiteren Techniken mit zunehmend komplexen Inhalten bis hin zu prozeduralen Abläufen trainiert werden. Auch hierfür stehen Simulatoren zur Verfügung und zudem zahlreiche sog. Wet-lab-Kurse mit unterschiedlichen Schwerpunkten. Die Ausbildung im OP sollte parallel dazu schrittweise erfolgen, was bedeutet, dass Eingriffe mit ansteigendem Anforderungsgrad zunächst in Teilschritten und letztlich in Gänze durchgeführt werden.
