Samenvatting
Doel: Vaststellen of bij fracturen aan de extremiteiten peroperatieve driedimensionale beeldvorming, 3-Dimensional Rotational X-ray (3D-RX), meer inzicht verschaft in de positie van de fractuurfragmenten en het osteosynthesemateriaal dan het peroperatieve gebruik van tweedimensionale (2D) röntgendoorlichting. Met de 3D-RX-scan kunnen tweedimensionale opnamen van de botstructuren worden gemaakt, maar ook een driedimensionale reconstructie die vergelijkbaar is met een CT-scan.
Opzet: Retrospectief cohortonderzoek.
Methode: In een periode van 2,5 jaar werden 85 fractuuroperaties geïncludeerd. Vastgesteld werd bij hoeveel operaties een correctie in de repositie en/of fixatie werd aangebracht naar aanleiding van de peroperatieve 2D-röntgendoorlichting, en/of bij hoeveel operaties correcties werden verricht na (additioneel) gebruik van een 3D-RX-scan, terwijl er geen indicatie was voor deze correcties naar aanleiding van de 2D-röntgendoorlichting.
Resultaten: Bij twintig van de 85 fractuuroperaties (24%) werd een correctie aangebracht na 2D-röntgendoorlichting; een aanvullende 3D-RX-scan gaf bij twee van deze twintig patiënten aanleiding tot het verrichten van extra correcties. Bij de overige 65 fractuuroperaties gaf de 2D-röntgendoorlichting geen aanleiding tot correcties; een additionele 3D-RX-scan leidde bij elf van deze patiënten toch tot correcties in de repositie en/of fixatie. In totaal werden dus bij dertien van de 85 patiënten (15%) aanvullende correcties verricht na een 3D-RX-scan, naar aanleiding van een probleem dat niet was herkend op de eerder gemaakte 2D-röntgendoorlichting.
Conclusie: Peroperatieve 3D-beeldvorming verschaft, ten opzichte van de 2D-röntgendoorlichting, meer informatie bij 15% van de fractuuroperaties aan de extremiteiten. Deze informatie biedt de chirurg de gelegenheid om de repositie en/of fixatie nog tijdens de operatie te verbeteren.
Abstract
Aim: To determine whether the intra-operative use of 3-Dimensional Rotational X (3D-RX) ray in fractures of the extremities improves insight in the position of fracture fragments and fixation material when compared to conventional 2D-fluoroscopic images. In addition to conventional 2D-fluoroscopic images, 3D-RX-ray can produce a 3D-reconstruction of the bony structures in different directions, comparable to Computer-assisted Tomography.
Study type: Retrospective cohort study.
Method: In a 2.5-year period, 85 operations on fractures of the extremities were included. In this study we determined the number of operations in which corrections in reduction and/or fixation were performed after 2D-fluoroscopy, and/or a 3D-RX-scan. Both imaging modalities were used for control purposes. In addition, we determined the number of operations in which corrections were performed after a 3D-RX-scan while there was no indication for these corrections based on the previous intra-operative 2D-fluoroscopic images.
Results: In 20 of the 85 operations (24%) corrections were performed after 2D-fluoroscopy. The additional 3D-RX-scan gave reason to perform extra corrections in 2 patients. In the 65 remaining operations in which no corrections were performed after 2D-imaging, the 3D-RX-scan prompted the surgeon to perform corrections in 11 patients. Consequently, in 13 out of 85 operations (15%) additional corrections were performed after a 3D-RX-scan, because the problem had not been recognized on previous performed intra-operative 2D-fluoroscopy.
Conclusion: An intra-operative 3D-RX-scan provides more information to the surgeon than 2D-fluoroscopy in 15% of the fracture operations of the extremities. This gives the surgeon the opportunity to optimize fracture reduction and/or fixation during the operation.
