Zusammenfassung
Im vorliegenden Artikel berichten wir über unsere Erfahrungen bei der Behandlung von Knochendefekten an der oberen Extremität mit vaskularisierten Knochentransplantaten.
Unter Berücksichtigung der aktuellen Literatur und aus unserer eigenen klinischen Erfahrung stellt die freie, vaskularisierte Knochentransplantation bei ausgedehnten Knochendefekten der oberen Extremität mit Weichteilbeteiligung die bestmögliche Behandlungsoption dar. Alternative, technisch weniger aufwändige Methoden haben keine nachweisbaren Vorteile. Komplikationen treten häufiger auf und die Behandlungsdauer ist länger als bei einer optimal durchgeführten mikrochirurgischen Rekonstruktion.
Abstract
This article reports on our experience treating bone defects of the upper extremity with vascularized bone grafts. In consideration of the current literature and our own experience, free vascularized bone grafts provide the best possibility to treat extensive bone defects that involve soft tissue of the upper extremity. Alternative, less challenging methods show no explicit advantages. Complications occur more often and the duration of treatment is even longer compared to a microsurgical reconstruction performed by an experienced surgeon.
