Zusammenfassung
Einleitung
Vor der Implantation endovaskulärer Aortenstents ist das Mehrzeilen-Angio-CT heute akzeptierter Standard der präoperativen Planung. Mit Hilfe multiplanarer Rekonstruktionen der CT-Daten werden Längenausdehnungen und Gefäßdurchmesser an den Verankerungspunkten achsengerecht in Bezug auf die zu implantierende Prothese bestimmt. Bis vor kurzem noch war diese Form der medizinischen Bildbearbeitung fast ausschließlich Spezialisten mit teuren Lösungen vorbehalten.
Methoden
OsiriX ist ein kostenloses (sog. Open-Source-)Bildbearbeitungsprojekt für medizinische Bilder, das von Osman Ratib (Los Angeles) und von Antoine Rosset (Universität Genf) entwickelt wurde. OsiriX verbreitet sich derzeit rasch neben kostenpflichtigen und herstellergebundenen industriellen Systemen als freier Standard der medizinischen Bildbearbeitung über alle Fachdisziplinen hinweg. Die Software wurde deshalb hinsichtlich der Funktionalität als Planungssoftware zur achsengerechten Vermessung von Aortenstentgrafts in unserer Klinik evaluiert.
Ergebnisse
Unter Mac OS X bietet OsiriX dem Anwender die Funktionalität radiologischer Workstations. OsiriX arbeitet mit DICOM-Bildern verschiedener Gerätehersteller und ist einfach in ein vorhandenes PACS zu integrieren. Multiplanare 2D- und 3D-Rekonstruktionen der Gefäßverläufe von Dünnschicht-CTs sind schnell und einfach möglich und erlauben eine exakte achsengerechte Längen- und Durchmesserbestimmungen zur Planung von Aortenstentgrafts. Der Zeitaufwand für die Vermessung der thorakalen Aorta beträgt 7:40±0:55 min, für die abdominelle Aorta 10:15±1:04 min. Die Dokumentation der Messungen ist als Ausdruck, in OsiriX selbst oder durch Export in das PACS-System der Klinik möglich. Limitierend für die Anwendung kann die Hardwareausstattung des Systems bei Verwendung vieler CT-Schichten werden. Wesentliche Einschränkung ist die nicht vorhandene CE-Zertifizierung der freien Version.
Schlussfolgerung
Der Gefäßchirurg erhält mit OsiriX eine kostengünstige und professionelle medizinische Bildbearbeitungslösung zur Planung von Aortenstentgrafts, die allerdings an Hardware der Firma Apple gebunden ist. Wesentliche Anforderungen an ein System zur präoperativen Planung von Aortenstentsgrafts sind erfüllt. Ohne CE-Zertifizierung liegt die Verantwortung für den Einsatz jedoch beim einzelnen Benutzer.

Introduction
Computed tomography angiography (CTA) is an accepted standard for the pre-procedural planning of endovascular aortic stent graft repair (EVAR). Three-dimensional reconstruction and analysis of the computed tomography data set is enormously helpful and essential for proper sizing of length and diameter of the stent graft. Until recently this form of medical imaging was almost exclusively done by specialists with expensive hardware and software solutions.
Methods
OsiriX is a free (open-source) multidimensional image navigation and display software designed for display and interpretation of large sets of medical images such as multislice CTA studies, developed by Osman Ratib (Los Angeles) and Antoine Rosset (Geneva). This article discusses the suitability of the OsiriX software for sizing and planning EVAR in a clinical setting.
Results
Using the MacOS X operating system the user of OsiriX gets a full feature DICOM workstation for imaging and image processing. Due to the DICOM standard, integration of OsiriX into an existing PACS is easily possible. Multi-planar 2D and 3D reconstruction of vessels in a CTA is fast and simple and permits accurate axis-related lengthening and diameter measurements for the planning of EVAR. The average time for measurement of the thoracic aorta is 7:40±0:55 min and 10:15±1:04 min for the abdominal aorta. The documentation of measurements is possible as a printout in OsiriX or by export into the PACS system of the hospital. The use of many CT layers can be a limiting factor for the application of OsiriX depending on the hardware equipment. A substantial restriction is the lack of CE certification of the free version.
Conclusion
Using OsiriX vascular surgeons have access to an economical and professional medical imaging workstation for planning of EVAR, which presupposes hardware of the Apple Company. Substantial requirements of a system for the planning of EVAR are fulfilled but without CE certification the responsibility for the usage has to be taken by the individual user.
