Zusammenfassung
Das Bauchaortenaneurysma (BAA) ist eine Erkrankung des älteren Menschen. Bei 90% der Patienten ist die infrarenale Aorta befallen. 80% der Bauchaortenaneurysmen werden als Zufallsbefund bei der Sonographie des Abdomens diagnostiziert. Im asymptomatischen Stadium ist das Verhältnis zwischen Behandlungsrisiko und Rupturrisiko für die Operationsindikation maßgeblich. Operationsindikation besteht bei einem maximalen Querdurchmesser von ≥5 cm, sofern der Gesundheitszustand des Patienten eine operative Rekonstruktion gestattet sowie bei Patienten, die — mit oder ohne Ruptur — Syptome zeigen. Risikofaktoren, die das Auftreten eines Aneurysmas begünstigen, sind arterielle Hypertonie, Diabetes mellitus, Nikotinkonsum, Hyperlipidämie sowie Alter und Geschlecht. Die Wahrscheinlichkeit einer Ruptur steigt mit zunehmender Aneurysmagröße.
Operationstechnisch stehen heute zwei Behandlungsverfahren zur Verfügung: die offene Interposition einer Kunststoffprothese und die endovaskuläre Implantation eines Stentgrafts. 30-Tage-Letalität, Frühkomplikationsrate und Dauer des stationären Aufenthalts liegen signifikant höher bei den Patienten, die konventionell operiert werden, während bei der endovaskulären Methode das Auftreten von Spätkomplikationen und die Notwendigkeit von Reinterventionen bislang häufiger ist. Lebensqualität, Patientenkomfort und kürzere Rekonvaleszenzzeit sind weitere Argumente für die endovaskuläre Methode. Zurzeit laufende prospektiv-randomisierte Studien werden zeigen, ob die Langzeitergebnisse der endovaskulären Behandlung den Einsatz dieser Technik rechtfertigen.
Abstract
Abdominal aortic aneurysm (AAA) is generally a disease of elderly patients, with the infrarenal segment of the aorta being affected in 90% of cases. In 80% of the patients, the diagnosis is accidentally ascertained by ultrasonography of the abdomen. The indication for surgery in the asymptomatic stage is based on the relationship between risk of rupture and risk of treatment. Indications for treatment is given at a diameter of more than 5 cm if the condition of the patient allows, in symptomatic patients or ruptured AAA. Risk factors are hypertension, diabetes mellitus, smoking, hyperlipidemia, age and gender. The risk of rupture rises with increasing diameter.
Currently, there are two competing operative techniques, conventional operation and endovascular repair. The 30 days mortality, early complication rate and length of hospital stay are significantly higher in patients treated with open aneurysm repair, whereas up to now the rate of late complications is higher after endovascular AAA repair and the need for reintervention is more common. Quality of life, patient convenience and reduced time of convalescence are the criteria supporting the endovascular procedure. Ongoing prospective randomized studies focused on long-term results will verify the value of endovascular abdominal aneurysm repair.
