Zusammenfassung

Der Rheumapatient, mit seiner Tendenz zur multiplen Gelenkzerstörung und den fehlenden Kompensationsmöglichkeiten der Nachbargelenke profitiert entscheidend von einer stabilen und mehr noch zusätzlich mobilen Handgelenkssituation, um eine feinmotorische Gebrauchsfähigkeit der Hand zu gewährleisten. So stehen mobile Lösungen für das Handgelenk als Resektionsverfahren und prothetische Versorgung als rekonstruktive Lösungen zur Verfügung. Trotz der guten Erfolge in den modernen medikamentösen Therapien bestehen nur bestimmte Zeitfenster, in denen die adäquaten Therapien erfolgversprechend sind.

Abstract

For patients with rheumatoid arthritis preventive and reconstructive surgery of the hand provide better functional results and higher patient satisfaction when surgery is done adequately before the inflammatory stage, especially when multiple joints are affected. Synovectomy, arthrodesis and prosthetic reconstruction are able to guarantee maintenance of function even in late stages and severe destruction, when modern drug therapy cannot prevent further inflammatory attacks and increasing destruction.
