Zusammenfassung
Der Gichttophus zeigt in der Magnetresonanztomographie ein charakteristisches Signalverhalten. Insgesamt imponiert er in den T1-gewichteten SE-Sequenzen als muskelisointense Raumforderung. Dem histologischen Aufbau entsprechend ist die proteinreiche amorphe Matrix des Tophuskerns in den T2-gewichteten TSE-Sequenzen extrem hyperintens. Die in der Matrix abgelagerten signallosen Uratkristalle sind bildmäßig bedeutungslos. Das den Tophuskern umgebende gefäßreiche Granulationsgewebe hingegen erscheint in den T1-gewichteten SE-Sequenzen nach i. v.-Applikation von Gadolinium-DTPA hyperintens. Inflammierte Tophi zeigen ein deutliches hyperintenses Umgebungsödem. Intraossäre Tophi sind in der Magnetresonanztomographie infolge direkter Darstellung früher nachweisbar als im Röntgenbild oder imponieren größer. Vom Tophus erfaßte anatomische Strukturen wie Sehnen und Bänder sind hinsichtlich Verlagerung oder Destruktion gut beurteilbar. An den kleinen peripheren Gelenken ist in dieser Frage die axiale Schichtebene optimal geeignet.
Summary
The appearance of gouty tophus in magnetic resonance imaging (MRI) is characteristic. On T1- and T2-weighted SE images, the signal intensity of tophaceous lesions is similar to that of muscles. According to the histology, T2-weighted SE images demonstrate extremely hyperintense signals, which reflect the high protein content in the amorpheous center of the tophus. The microscopic urate crystals deposited there have no MRI signal and are of no further diagnostic impact. Vascularized granulation tissue surrounding the tophus center enhance after intervenous application of contrast agents (Gadolinium). The inflammed tophus is associated with local edema, causing high signal intensity. MRI is superior to plain radiography for early detection of intraosseous tophi. Involvement of anatomical structures such as ligaments and tendons can be evaluated sufficiently. For peripheral joints, axial slice orientation is most helpful.
