Arthroskopie des oberen Sprungelenks Eine retrospektive Analyse der Komplikationen

Zusammenfassung

Ziel unserer retrospektiven Studie war es, die Komplikationen unserer arthroskopischen Operationen am oberen Sprunggelenk (OSG) zu analysieren. Bei allen Patienten wurden die Operations- und Nachbehandlungsprotokolle ausgewertet, bei 111 Patienten (79,3%) erfolgte außerdem nach durchschnittlich 20,7 Monaten eine klinische und radiologische Nachuntersuchung. In unserem Patientenkollektiv fanden wir bei insgesamt 14 Patienten Komplikationen. Es handelte sich dabei 4-mal um eine Wundheilungsstörung, 2-mal um eine oberflächliche Infektion, 3-mal um eine tiefe Infektion, 3-mal um eine Läsion des N. cutaneus dorsalis intermedius und 2-mal um eine Phlebothrombose. Unsere Studie zeigt, dass bei detaillierter Kenntnis und Beachtung der Anatomie, einer exakten präoperativen Diagnostik und Planung sowie einer subtilen Präparation der Zugangswege die Komplikationsrate gesenkt werden kann. Insgesamt führt auch die Verwendung eines Holmium-Yag-Lasers für die arthroskopische Sprunggelenkchirurgie tendenziell zu einer Senkung der Komplikationsrate.

Abstract

The purpose of this retrospective study was to analyse the complications of our arthroscopic procedures at the ankle joint. In all patients we evaluated the treatment protocols and in 111 patients (79,3%) we also evaluated the clinical and radiologic results. All in all we have found complications in 14 patients. In 4 cases we have seen a delayed wound healing, in 2 cases a superficial infection, in 3 cases a deep infection, in 3 cases a neural damage and in 2 cases a phlebothrombosis. Our study show, that the complication rate could be minimized by detailed knowledge of the anatomy, exact preoperative diagnostic and planing of the operation and careful preparation of the portals. The use of a laser also shows a tendency to a lower complication rate.
