Zusammenfassung
Hintergrund
Goldstandard der operativen Therapie vorderer Kreuzbandrupturen ist die Rekonstruktion mittels Sehnentransplantat.
Fragestellung
Die dynamische intraligamentäre Stabilisierung des vorderen Kreuzbandes ist eine neue kreuzbanderhaltende Operationstechnik.
Material und Methoden
In der vorliegenden Arbeit werden die Indikationen, die Operationstechnik, die Nachbehandlung und vorläufige Ergebnisse nach erster Anwendung in Deutschland nach dynamischer intraligamentärer Stabilisierung des vorderen Kreuzbandes dargestellt.
Ergebnisse
Vierundzwanzig Frauen und 31 Männer mit akuter Ruptur des vorderen Kreuzbandes und einem Durchschnittsalter von 30,4 Jahren wurden in eine prospektive klinische Studie eingeschlossen. Von 26 Patienten konnten bereits Ergebnisse nach einem Follow-up von einem Jahr erhoben werden. Neben einer hohen subjektiven Patientenzufriedenheit ergaben die Outcomescores Lysholm, Tegner und International Knee Documentation Committee (IKDC) nach 12 Monaten gute Werte. Der Lachman-Test zeigte nach 12 Monaten im Mittel eine Differenz zur gesunden Seite von 1,7 mm.
Schlussfolgerungen
Die dynamische intraligamentäre Stabilisierung des vorderen Kreuzbandes kann in Verbindung mit der Mikrofrakturierung der Notch die biomechanischen und biologischen Voraussetzungen für eine primäre Heilung des vorderen Kreuzbandes schaffen. In weiteren Studien müssen die für diese Technik geeigneten Patienten bzw. die geeignete Rissform differenziert und die klinischen Langzeitergebnisse beobachtet werden.
Abstract
Background
Reconstruction of tears in the anterior cruciate ligament with a tendon graft is the current gold standard.
Objectives
Dynamic intraligamentary stabilization is a new technique for preservation of the anterior cruciate ligament.
Methods
This article describes the indications, operative technique, rehabilitation and preliminary results after dynamic intraligamentary stabilization for acute anterior cruciate ligament ruptures.
Results
A total of 24 women and 31 men with an acute anterior cruciate ligament tear were included in a prospective clinical trial. Of the patients 26 had already been followed-up for 12 months and satisfying values for the Lysholm, Tegner and International Knee Documentation Committee (IKDC) outcome scores were achieved. High subjective patient satisfaction was also achieved. The Lachman test showed a mean anterior translation difference to the healthy side of 1.7 mm.
Conclusion
Dynamic intraligamentary stabilization in combination with microfracturing of the notch can provide biomechanical and biological conditions for self-healing of the anterior cruciate ligament. Further clinical and biomechanical research is needed to identify appropriate patients and rupture types suitable for this new technique.
