Zusammenfassung
Die Frakturen der lateralen Klavikula machen insgesamt einen Anteil von 12–15% aller Klavikulafrakturen aus. Im Gegensatz zum eindeutigen Behandlungsregime bei den wesentlich häufigeren Frakturen des mittleren Drittels werden die Therapieoptionen bei den peripheren Frakturen in der einschlägigen Literatur kontrovers diskutiert. Die hohe Pseudarthroserate bei diesen Frakturen von z. T. über 40% zeigt, dass sie sich biomechanisch von denen des mittleren und sternalen Drittels unterscheiden. Die korakoklavikulären Bänder spielen dabei eine ähnlich entscheidende Rolle wie bei der Akromioklavikulargelenksprengung. Um eine sichere Therapieentscheidung treffen zu können, bedarf es zunächst einer behandlungsorientierten Klassifikation. Hier kommt die Einteilung nach Jäger und Breitner zur Anwendung, in der 4 unterschiedliche Frakturtypen voneinander unterschieden werden. Die Therapieoptionen der einzelnen Typen reichen dabei von konservativen Verfahren bis zu unterschiedlichen Operationstechniken. Der Beitrag soll einen Handlungsleitfaden zur sicheren Diagnostik und Behandlung dieses besonderen Frakturtyps bieten.
Abstract
Fractures of the lateral clavicle end account for 12–15 percent of all clavicle fractures. In contrast to the clear treatment of midshaft fractures the therapy of the distal third is still open to controversy. The high non-union rate up to 40 percent that occurs with the lateral end fractures shows the special biomechanical mechanisms. The role of the coracoclavicular ligaments is comparable to the acromioclavicular joint disruption. To make a clear decision about the therapy we need a treatment based classification such as the one by Jäger and Breitner, which distinguishes four different fracture types. The treatment options reach from conservative therapy up to numerous different operative techniques. The aim of this article is to demonstrate a simple and save way for the diagnosis and therapy of that special kind of fracture.
