Zusammenfassung
Hintergrund
Gefäßresezierende Eingriffe und die damit verbundene Notwendigkeit der Rekonstruktion zur Aufrechterhaltung insbesondere der Leber- und Dünndarmperfusion sind v. a. in der chirurgischen Therapie des Pankreaskarzinoms von Bedeutung. Die R0-Resektion ist dabei der einzig kurative Therapieansatz. Eine Infiltration des Tumors in venöse oder arterielle Gefäße und die damit verbundene Resektion und Rekonstruktion zur vollständigen Tumorentfernung und Sicherstellung einer ausreichenden Perfusion der abhängigen Organe stellt eine der größten Herausforderungen in der Pankreaschirurgie dar. Zudem ist die onkologische Wertigkeit – im Hinblick auf arterielle Gefäßresektionen – umstritten.
Fragestellung
In dieser Übersichtsarbeit werden – basierend auf der aktuellen Literatur – die Indikationen und technischen Aspekte venöser sowie arterieller Resektionen und Rekonstruktionen in der Therapie des Pankreaskarzinoms dargestellt und diskutiert.
Material und Methoden
Es wurde eine systematische Literaturrecherche in Medline, Embase und der Cochrane Library durchgeführt, um Studien zu identifizieren, in denen die Ergebnisse venöser oder arterieller Gefäßresektionen und -rekonstruktionen in der Pankreaschirurgie vorgestellt werden.
Ergebnisse
Ein Pankreaskarzinom mit Gefäßinfiltration ist nicht prinzipiell als irresektabel anzusehen, sondern muss in jedem Fall auf die Möglichkeit einer kurativen Resektion überprüft werden, wobei ein entscheidender Faktor die Differenzierung zwischen venösen und arteriellen Gefäßbeteiligungen ist. Für die venöse Gefäßresektion stehen abhängig vom Ausmaß des Tumorbefalls verschiedene sichere technische Optionen bereit. Arterielle Gefäßresektionen sind mit einer erhöhten Morbidität und Mortalität verbunden. Bei selektionierten Patienten kann durch eine arterielle Gefäßresektion eine vollständige Tumorresektion und ein verlängertes Überleben erreicht werden.
Abstract
Background
Vascular resection interventions and the associated necessity of a reconstruction for maintenance particularly of hepatic and small intestinal perfusion are important aspects especially for the surgical treatment of pancreatic cancer. An R0 resection is the only curative treatment option for patients with pancreatic cancer. Venous or arterial vascular infiltration by the tumor and the associated resection and reconstruction for complete tumor removal and establishment of a sufficient perfusion of the dependant organs represents one of the greatest challenges in pancreatic surgery. In addition the oncological significance with respect to arterial vascular resections is controversial.
Objective
In this review article the indications and technical aspects of vascular resection and reconstruction in the therapy of pancreatic cancer are presented and discussed based on the current literature.
Material and methods
A systematic search of Medline, Embase and the Cochrane Library was carried out to identify studies reporting the results of venous or arterial vascular resection techniques, postoperative morbidity, mortality and patient survival after surgery for pancreatic cancer.
Results
Pancreatic cancer with vascular infiltration should not principally be seen as non-resectable but must always be checked for the possibility of a curative resection. A decisive factor is the differentiation between venous and arterial vascular involvement. Various safe technical options are available for venous vascular resection, depending on the extent of tumor infiltration. Arterial vascular resections are associated with an increased morbidity and mortality. In selected patients a complete tumor resection and prolonged survival can be achieved by arterial vascular resection.
