Stellung der Strahlentherapie in der Behandlung der Knochentumoren

Zusammenfassung

Primäre maligne Knochentumoren treten relativ selten auf.Die häufigsten Knochenläsionen sind das Osteosarkom,das Ewing-Sarkom,das Chondrosarkom,das Fibrosarkom, das maligne fibröse Histiozytom des Knochens,Riesenzelltumoren, aneurysmatische Knochenzysten und das Chordom.Im Allgemeinen gelten diese Knochentumoren als weitgehend strahlenresistent.Jedoch sollte dies nicht zu einem therapeutischen Nihilismus oder frustranen Resektionsversuchen bei inoperablen Tumoren führen,da gezeigt wurde, dass die Strahlentherapie auch bei diesen Tumoren gute palliative z.T. auch kurative Ergebnisse erzielen kann, wenn eine ausreichende Dosis und ein adäquates Volumen bestrahlt wird.Lediglich beim Ewing-Sarkom hat die Strahlentherapie einen festen Platz in der Primärbehandlung im Rahmen der multimodalen Therapiestrategie. Die häufigsten malignen Knochentumoren und die Rolle der Strahlentherapie bei deren Behandlung werden im Folgenden diskutiert.

Abstract

Primary malignant bone neoplasms are relatively rare.The most common bone tumors are osteosarcoma,Ewing's sarcoma,chondrosarcoma, fibrosarcoma,malignant fibrous histiocytoma of bone, giant cell tumor, aneurismal bone cyst and chordoma.These tumors are generally considered to be a radioresistant entities, but it has been suggested that radiotherapy may be effective in a palliative and in some curative situations, if a sufficient dose is given to an adequate volume. Only for the management of primary Ewing's sarcoma the radiation therapy is an essential part in the multimodal therapy concept.The most common bone neoplasms and the role of the radiotherapy are discussed in these chapter.
