Zusammenfassung
Muskelrelaxanzien sind der häufigste Auslöser allergischer Reaktionen in der Anästhesie. Es wird über den Fall einer lebensbedrohlichen anaphylaktischen Reaktion bei Narkoseeinleitung berichtet, bei der sich als Erstsymptom der Anaphylaxie ein schwerer Bronchospasmus fand. Eine Beatmung war zeitweilig nahezu unmöglich. Angesichts der Schwere der Reaktion waren eine multimodale antiallergische Therapie und eine hoch dosierte Katecholamintherapie erforderlich. Als Auslöser der anaphylaktischen Reaktion konnte das Muskelrelaxans Rocuronium mithilfe des Intrakutantests identifiziert werden.
Abstract
Muscle relaxant drugs are the most frequent cause of anaphylactic and anaphylactoid reactions during anaesthesia. We report a case of a life-threatening anaphylactic reaction during induction of anaesthesia with severe bronchospasm as the first clinical symptom. Mechanical ventilation was nearly impossible. The patient required a multimodal antiallergic therapy and a high-dose catecholamine therapy for stabilization. Rocuronium was identified as the allergic agent using intradermal testing.
