Arthrose des medialen Kniekompartments.
Überlastung des medialen Kniekompartments.
Genu varum.
Kontraindikationen
Nikotinabusus.
Gefäßerkrankung.
Schlechte Weichteildeckung.
Operationstechnik
Gerade, 12 cm lange Inzision, beginnend distal und medial der Tuberositas tibiae, dann parallel zur Tibiavorderkante verlaufend. Schräg im Winkel von 60° angelegte Osteotomie von distal-medial nach proximal-lateral. Erhaltung der lateralen Kortikalis. Öffnende Osteotomie und Kontrolle der Beinachse mittels Diathermiekabel. Zwei trikortikale Beckenspäne werden aus dem Beckenkamm der gleichen Seite entnommen und in den Osteotomiespalt eingepasst. Alternativ kann auch ein Knochenersatzstoff verwendet werden. Stabilisierung über eine angeformte T-Platte. Drainage und Wundverschluss.
Ergebnisse
44 Patienten (32 Männer, zwölf Frauen, Durchschnittsalter 48 Jahre) mit 45 Osteotomien. Nachuntersuchungszeitraum 30 Monate (7–74 Monate). Alle durchgeführten Osteotomien führten zur Ausheilung. 90% der Patienten zeigten nach dem HSS (Hospital for Special Surgery) Knee Score exzellente Ergebnisse. In 77% wurde der femorotibiale Winkel um 5–12° korrigiert. Die Patienten erreichten ihr präoperatives Bewegungsausmaß. Es traten keine schweren Komplikationen auf.
Abstract
Objective
Restoration of mechanical axis of the leg.
Indications
Osteoarthritis of medial knee compartment.
Overload of medial compartment.
Genu varum.
Contraindications
Smoker.
Vascular impairment.
Poor soft-tissue envelope.
Surgical Technique
Straight 12-cm midline incision starting distal to the tibial tubercle medially and continuing parallel to the tibial crest. Oblique osteotomy at 60° distal-medial to proximal-lateral preserving the lateral cortex. Osteotomy wedged open and alignment checked with diathermy cord. Two tricortical bone blocks harvested from ipsilateral iliac crest or bone substitute wedges placed in osteotomy. Stabilization with contoured T-plate. Closure over drain.
Results
44 patients (32 men, twelve women, average age 48 years) with 45 osteotomies. Follow-up 30 months (7–74 months). All osteotomies consolidated. 90% patients had excellent results according to the HSS (Hospital for Special Surgery) Knee Score. In 77% the femorotibial angle was corrected to 5–12°. Patients regained their preoperative range of motion. There were no major complications.
