Zusammenfassung.
Hintergrund: Aufgrund ihrer Seltenheit sind bei primären malignen Lymphomen der Leber bislang keine standardisierten therapeutischen Konzepte entwickelt worden.
Fallbericht: Wir berichten über den Fall eines diffusen großzelligen B-Zell-Lymphoms der Leber bei einer 48-jährigen Frau. Es handelt sich um eine solitären Tumor, dessen maximaler Durchmesser 12,5 cm betrug. Nach neoadjuvanter Chemotherapie konnte das Lymphom vollständig reseziert werden. Histologisch zeigte sich eine hochgradige therapieinduzierte Tumorregression. Eine Infiltration regionärer Lymphknoten oder anderer Organe war im Rahmen des Stagings nicht erkennbar (Stadium I EA). Nach sechs Zyklen einer adjuvanten Chemotherapie ist die Patientin seit mehr als 5 Jahren beschwerdefrei, ohne dass sich Hinweise auf ein Rezidiv des Leberlymphoms ergeben haben.
Schlussfolgerung: Der vorliegende Fall dokumentiert den Effekt einer systemischen Chemotherapie. Daher wird sie in vielen Zentren als alleinige Therapie eingesetzt. Die neoadjuvante Chemotherapie stellt jedoch möglicherweise eine wichtige Ergänzung der therapeutischen Strategien bei unilokulärem primärem Leberlymphom dar.
Abstract.
Background: Since primary malignant lymphomas of the liver represent a rare entity, no commonly accepted therapeutic strategy has been developed so far.
Case Study: We report the case of a diffuse large-cell B-cell lymphoma of the liver in a 48-year-old female patient. The tumor presented as a solitary mass measuring 12.5 cm in maximum diameter. After neoadjuvant chemotherapy, the lymphoma was completely resected (R0). A massive therapy-induced tumor regression was found histologically. No involvement of regional lymph nodes or other organs was detected during staging procedures (stage I EA). After six courses of adjuvant chemotherapy, the patient has been alive and well for more than 5 years and shows no evidence of tumor relapse.
Conclusion: This case documents the effect of systemic chemotherapy on lymphoma cells. In many centers systemic chemotherapy is used as the only therapeutic regimen. Neoadjuvant chemotherapy might, however, represent an important addition to the therapeutic strategies concerning unilocular primary hepatic lymphomas.
