Die Tumoren im Foramen occipitale magnum

Zusammenfassung

Es wird über eine in den letzten 20 Jahren beobachtete Serie von 37 Fällen mit Tumoren berichtet, die zumindest teilweise im Bereich des Foramen occipitale magnum lokalisiert waren. Die klinischen Symptome, die durch die Lage und Größe der Geschwulst bestimmt sind, werden analysiert. Die Diagnose wurde jeweils durch zusätzliche Jodölventrikulographie, seltener auch durch Vertebralisangiographie verifiziert. Unter den Tumoren fanden sich 12 extrazerebrale, 24 intraparenchymale und 1 Karzinommetastase. Von den 12 extrazerebralen Geschwülsten konnten 10 radikal entfernt werden, ohne Mortalität. Bei den übrigen war nur eine partielle Entfernung oder eine Dekompression ausgeführt worden. Davon starben 6 Fälle.

Die Ergebnisse der Analyse des eigenen Krankengutes werden mit den Angaben des Schrifttums korreliert.

Bei den Tumoren des Foramen occipitale magnum ist der Versuch der operativen Entfernung immer angezeigt, sofern nicht aus anderen Gründen sich jegliche Operation verbietet. Die Radikalexstirpation bringt die besten operativen Ergebnisse. Sie sollte deshalb angestrebt werden.

Summary

A series is reported of 37 patients seen during the last 20 years with tumours which lay at least partially in the region of the foramen magnum. An analysis was made of the clinical symptoms which were determined by the position and size of the tumour. The diagnosis was verified at times by an associated myodil ventriculography, and occasionally also by means of vertebral angiography. The tumours comprised 12 extracerebral lesions, 24 intrinsic lesions and 1 metastatic carcinoma. Ten of the 12 extracerebral tumours could be radically excised, and there were no deaths. With the rest of the tumours only partial removal or a decompression could be carried out, and six of these patients died.

The results of the analysis of the individual case records were correlated with the data of the literature.

With tumours of the foramen magnum an attempt to remove the lesion surgically is always advisable insofar as it is not contraindicated on other grounds in each individual operation. Because radical extirpation produces the best operative results one should therefore strive to achieve it.
