Zusammenfassung
Bei 56 Patienten der Klasse II wurde die funktionelle Anatomie der Nase bei Patienten mit offenem und abgestütztem Biß rhinoskopisch und rhinomanometrisch untersucht. Bei Patienten mit offenem Biß wurde eine mechanische Einengung der Nase häufig beobachtet, während vergrößerte Adenoide als Ursachenfaktor einer behinderten Nasenatmung von untergeordneter Bedeutung waren. Die Verteilung der rhinoskopischen Befunde stand in Abhängigkeit zur kraniofazialen Morphologie in der Vertikalen. Rhinomanometrisch war der Nasenwiderstand bei Patienten mit offenem Biß und vertikalem Gesichtsschädelaufbau am größten. Ein Abschwellen der Mukosa zeigte, daß eine behinderte Nasenatmung vor allem-auf eine Schleimhautschwellung zurückzuführen war. Bei kleiner Volumenzuwachsrate nach Abschwellen der Mukosa wurde häufiger die Ausbildung eines offenen Bisses beobachtet. Der Befund war unabhängig von der kraniofazialen Morphologie in der Vertikalen. Die Ergebnisse belegen, daß beim offenen Biß die Genese einer behinderten Nasenatmung auf drei Faktoren beruhen kann: 1. Verlegung der Choanen durch Adenoide, 2. funktionelle Behinderung durch Isthmusstenose und 3. erhöhter Nasenwiderstand durch Turbulenzen des Nasenstroms. Die therapeutischen Möglichkeiten aus HNO-ärztlicher und kieferorthopädischer Sicht werden diskutiert.
Summary
In 56 class II cases, functional anatomy of the nose in subjects with open bite and overbite was studied rhinoscopically and by means of rhinomanometry. In patients with open bite a mechanical obstruction of the nose was frequently observed, enlarged adenoids having little significance as a causative factor. Rhinoscopical findings were closely related to the vertical facial morphology. Nasal resistance was highest in patients with open bite and vertical growth pattern. Decongestion of the mucosa revealed that nasal obstruction was mainly due to mucosal swelling. In patients with a minor decrease of nasal resistance after decongestion, the incidence of open bite was increased. Here, no correlations were found to facial morphology in the vertical plane. According to our results, nasal obstruction in cases of open bite may be due to: 1) obstruction adenoids, 2) nasal isthmus stenosis or 3) increased nasal resistance due to turbulent flow. The possibilities für E.N.T. and orthodontic treatment are discussed.
Résumé
Chez 56 patients de la classe II présentant soit une béance soit une supraclusion incisive, on a examiné l'anatomie et la fonction nasales. On a observé fréquemment un rétrécissement du conduit nasal chez les cas de béance alors que l'hypertrophie adénoïdienne est un facteur moins important. Les observations rhinoscopiques sont étroitement liées à la configuration crânio-faciale verticale. La perméabilité nasale est la moins bonne chez les patients présentant une béance associée à un type de croissance crânio-faciale à prédominance verticale. Le gonflement de la muqueuse nasale amène également une gêne respiratoire. La tendance à la béance augmente chez les patients qui ne montrent pas d'amélioration de la perméabilité nasale après la décongestion de la muqueuse: cette dernière observation n'a rien à voir avec le type crâniofacial vertical. En résumé l'obstruction de la perméabilité nasale en cas de béance est provoquée par: 1) l'obstacle adénoïdien, 2) l'obstruction fonctionnelle due au rétrécissement des choanes, 3) des perturbations au passage de l'air. En conclusion, les modalités de traitement sont à envisager tant au point de vue de la rhinologie que de l'orthopédie dento-faciale.
