Aseptic diaphyseal pseudarthrosis: Treatment by rigid internal fixation

Summary

The authors define their concepts with regard to nonunion of fractures and study the possible causes of aseptic diaphyseal pseudarthrosis in the 182 cases in their casuistics. They have found that the indication for osteosynthesis was incorrect in 34% of the cases and when it was correct, the technique was poor in 58%. The authors conclude that the risk of non-union in fractures and nonconsolidation of pseudarthrosis can be minimished if there is: (1) correct indication with respect to type and level of fracture or pseudarthrosis; (2) choice of an appropriate internal fixation method; and (3) good achievement of the selected osteosynthesis.

Zusammenfassung

Die Autoren legen ihre Auffassung von der pseudarthrotischen Heilung von Frakturen dar und untersuchen die möglichen Ursachen von aseptischen diaphysären Pseudarthrosen unter den 182 Fällen ihrer Kasuistik. In 34% der Fälle wurde eine Fehlindikation für die Osteosynthese und in 58% eine technisch mangelhafte Osteosynthese bei richtiger Indikation als Ursache für die Pseudarthrose ermittelt. Das Risiko einer pseudarthrotischen Frakturheilung bzw. der unterbleibenden Konsolidierung einer bereits bestehenden Pseudarthrose kann herabgesetzt werden, wenn folgende Bedingungen erfüllt werden: 1. Korrekte Indikation zur Osteosynthese unter Berücksichtigung des Typs und der Lokalisation der Fraktur bzw. der Pseudarthrose, 2. Wahl des angemessenen Osteosyntheseverfahrens, 3. gute Ausführung der gewählten Osteosynthese.
